Evaluation of antibody testing for SARS-Cov-2 using ELISA and lateral flow immunoassays

[..]
Results: ELISA detected SARS-CoV-2 IgM or IgG in 34/40 individuals with an RT-PCR-confirmed
diagnosis of SARS-CoV-2 infection (sensitivity 85%, 95%CI 70-94%), vs 0/50 pre-pandemic
controls (specificity 100% [95%CI 93-100%]). IgG levels were detected in 31/31 RT-PCR-
positive individuals tested ≥10 days after symptom onset (sensitivity 100%, 95%CI 89-100%).
IgG titres rose during the 3 weeks post symptom onset and began to fall by 8 weeks, but
remained above the detection threshold. Point estimates for the sensitivity of LFIA devices
ranged from 55-70%
versus RT-PCR and 65-85% versus ELISA, with specificity 95-100% and
93-100% respectively. Within the limits of the study size, the performance of most LFIA
devices was similar.
Conclusions: The performance of current LFIA devices is inadequate for most individual
patient applications. ELISA can be calibrated to be specific for detecting and quantifying SARS-
CoV-2 IgM and IgG and is highly sensitive for IgG from 10 days following symptoms onset.

Vitamin D Insufficiency is Prevalent in Severe COVID-19

Conclusions:
This small, retrospective observational study suggests a link between VDI and severe COVID-19. Anecdotal and observational data indicate that VDI may play a significant role in the progression of the COVID-19 disease state. Low-risk, high-reward potential therapies that target CAC and VDI merit further investigation. Prospective, randomized controlled studies that properly risk-stratify subjects should be performed

Patterns of COVID-19 Mortality and Vitamin D: An Indonesian Study

KEY FINDINGS:
• Majority of the COVID-19 cases with insufficient and deficient Vitamin D status died.•The odds of death was higher in older and male cases with pre-existing condition and below normal Vitamin D levels.
• When controlling for age, sex, and comorbidity, Vitamin D status is strongly associated with COVID-19 mortality.
• Randomized controlled trials are warranted to investigate the role of vitamin D supplementation on COVID-19 outcomes and to establish the underlying mechanisms.

Unfair to brand Chinese kits faulty, says embassy
NEW DELHI: After ICMR’s rejection of Chinese rapid test kits, China on Tuesday said it was unfair and irresponsible for “certain individuals” to label Chinese products as faulty and look at issues with “preemptive prejudice”.
Recalling statements by the manufacturers that the kits had been validated and approved by ICMR, the embassy here expressed hope that India will respect China’s “goodwill and sincerity” and strengthen communication with relevant Chinese companies “based on facts” to resolve the issue “reasonably”.

“We have also learned that there are strict requirements for the storage, transportation and use of Covid-19 antibody rapid test kits. Any operation which is not carried out by professionals in accordance with the product specifications will lead to testing accuracy variations. ICMR also made it clear that rapid antibody test kits should only be used for surveillance purposes,” the embassy spokesperson said.

Scientists are perplexed by the low rate of coronavirus hospitalizations among smokers. Nicotine may hold the answer.
Tim O'Donnell
The WeekApril 28, 2020, 12:02 PM CDT

No experts are remotely advocating for people to take up smoking to prevent COVID-19, but some researchers have theorized nicotine may be playing some role in keeping the virus at bay, Vice reports. That's because there's a surprisingly low rate of smokers among coronavirus hospitalizations.

In France, for example, 25 percent of the population smokes, but only 5.3 percent of coronavirus patients have been recorded as smokers, and studies have found low rates in China and New York City, as well.

Greek cardiologist and tobacco harm-reduction specialist Konstantinos Farsalinos thinks nicotine (crucially, not tobacco) might be lessening the intensity of cytokine storms, an overreaction of the body's immune system which seems to be the cause of the most severe coronavirus symptoms. French researchers have a slightly altered theory that nicotine prevents the virus from entering cells (the difference lies in the type of receptors the virus latches onto), and they're hoping to test out nicotine patches on patients to see if they help fight off COVID-19. The French government suspended the online sale of patches to make sure people don't buy in bulk and try to treat themselves that way.

The seemingly out-there theory has piqued the interest of scientists across the world, though many are urging caution. The lower rates could be a result of some other chemical in tobacco producing a protective effect, or it could be that the number of smokers is being underreported.

"Smokers who have developed chronic disease have likely quit because of their disease," Michael Siegel, a community health sciences professor at Boston University, said. "Many of the smokers who are continuing to smoke are doing so because they don't have disease yet. So this would be expected to skew the sample of hospitalized patients toward people who do not smoke." Read more at Vice.

COVID-19 Vaccine Likely By September, Says Indian Firm Partnering Oxford

A vaccine for coronavirus may be available in the market as early as the end of September, the chief of the world's largest vaccine maker has told NDTV. The cost, he said, would be roughly around Rs 1,000. "That's what we hope to do by the end of May- we hope to start producing this vaccine so that by the time the trials get over in September, we could have a product that we can give to the Indian people and the world and not have to wait another 6 months after that to manufacture the vaccine," Adar Poonawalla, the chief of Serum Institute of India, told NDTV on Monday.

This is some confidance. Even before trial they are into mass production.

After Bhaskar's revelations, the state government also inquired about the death toll through intelligence.

In ThePrint, Coupta-ji has some details on this, especially safety aspects and background. I guess if they miss out other vaccine makers would over take them. Also I think this could very well be like a yearly "flu" shot, which means a guaranteed revenue stream.

Not discounting the effect of nicotine, however need to be careful about claims on smokers not being effected.

Most of the time, people lie on paper that they don't smoke. Telling the truth drives up medical insurance...

Mate of mine, who heavily smoked in the recent time and thinks he was hit with the virus, was coughing up to 3 weeks. He said the cough didn't go, no matter what you do.

China is full of smokers, and they didn't draw any such correlation. In fact, it was said that smokers were more likely to be affected due to their reduced lung capacity/function.

"NewsX", if it hasn't been mentioned before, is a pro-Dharmic, pro-India TV channel. Add them to the list of Republic TV, Times Now, WION, India TV..

"India Today TV" is centrist, usually not stridently anti-BJP or anti-Dharmic. I would still watch it!

https://m.rediff.com/news/report/coronavirus-in-india-number-of-red-zones-decrease-but-orange-zones-up/20200429.htm

The Centre had on April 15 classified districts into three categories.

Those with high load of COVID-19 cases or which have a high growth rate of the disease are marked as hotspots or red zones, while those with considerably fewer cases of the respiratory infection fall in orange zone or non-hotspots.

The number of COVID-19 hotspot districts in India as on Wednesday morning has come down to 129 from 170 a fortnight ago,

but in the same period the number of infection-free districts or green zones too decreased from 325 to 307, according to official sources.

During this time, the number of non-hotspot districts, also known as orange zones, increased from 207 to 297, they said.

With single source/no source information- it is complicated to follow what is going on. I am afraid it is going to be worse if we allow for other events ( migrants international or between states etc ). For example Chennai was in the 10s or 20s ...now suddenly bigger numbers with no explanation

I was hoping for granularity at the level of tehsil/taluk or smaller etc to manage lockdowns efficiently.

The good thing so far as exit strategy is arogya setu and mandatory use. I wish the epidemic act is used/modified to really take over state govt atleast in part - If the states dont adhere to central health guidelines and transparency.

Atleast new cases should be identified as "previously traced/quarantined" or new

How can we say that HCQ or any other drug is effective when 80 to 90 percent infected patients recover on their own without being critical or requiring hospitalization. The recovery rate has to be significantly higher than 90% to claim effectiveness.

I am kinda disappointed right now that a graded relaxation from 3rd May has not been formally announced by the MH state government, for the general populace. Some customer's (engineering cos in Maharashtra, Jharkhand, MP) have started at 10% capacity.
My cousin owns a non-essentials store in a tier-2 city in Maharashtra, and he has no clue if and when to call his staff back (all from villages around), right now with 0 cash flow he cannot pay them anyway. I think all state governments are waiting for who announces or comes back with a plan first.
As reported by Dr.Ratnadip on this forum, Devendra Fadnavis has officially written to the govt about huge spike in burials in Malegaon, without a corresponding rise in official COVID numbers. There were numbers as well, something like close to 500 in couple of weeks instead of about 200 or so during previous months.

Covid-19: India virus deaths pass 1,000, but low toll puzzles experts
India's confirmed coronavirus death toll passed 1,000 on Wednesday following its highest daily increase, but the numbers remain low compared with Europe and the United States in a phenomenon that is puzzling experts.
Agence France-Presse, New Delhi, April 30, 2020

India's confirmed coronavirus death toll passed 1,000 on Wednesday following its highest daily increase, but the numbers remain low compared with Europe and the United States in a phenomenon that is puzzling experts.
With massive slums and a shaky healthcare system, there were fears India would be ravaged by the pandemic that has killed more than 214,000 people worldwide. The latest daily toll of 73 deaths was India's highest, offering a warning that the giant South Asian nation was not yet in the clear.
A lack of testing and many other factors mean that India's official toll of 1,007 deaths could be far below the real number of coronavirus victims.
"We see low numbers but we do not know how to validate those numbers or rates," virologist T. Jacob John told AFP.
"Governments desire under-reporting and... we are flying blind for true rates and numbers." India appears so far to have been spared the devastation seen in New York, Milan and other hard-hit parts of the world, where hospitals have been overwhelmed by cases of coronavirus.
Experts have offered a number of theories and factors, but there is no definitive explanation yet. "It might well be true that the trajectory of the Indian epidemic is very different for reasons that we do not understand... but those are all theories right now," Prabhat Jha, an epidemiologist at the University of Toronto, told AFP. One possible factor is that India imposed a lockdown on its 1.3 billion people on March 25, when there were 606 confirmed cases and 10 deaths, and it has been rigidly enforced.
